Complete this form and return it AS SOON AS POSSIBLE to insure you will get your choice of dates. Please schedule at least two weeks in advance. Arrival time at the Zoo should be 10:00 am. IF THIS IS NOT POSSIBLE, PLEASE INDICATE THE EARLIEST THAT YOU CAN ARRIVE.

Please note - We no longer need the names of the teachers who will be attending. We just ask that the students be divided into 2 equal groups if you have approximately 40-64 students and 3 groups if you have 65-100. The maximum number of students per day is 100. The teachers and
chaperones (one adult per every five students) should be equally distributed among the groups. The students and chaperones should know before they arrive if they are in “Group 1”, “Group 2” or “Group 3”.
